Referrals

Eligibility

• Children between the ages of 3 – 17
• Parent(s) must be registered under the palliative program
• Family consents to referral

Referral Process

• Have your healthcare provider (nurse, social worker, physician) contact us at bal@lastingmagic.org
• We will receive the required information and contact you directly
• A visit will be made by one of our nursing members (either by phone call or home visit)
• We will discuss your needs/wishes and help you use your imagination
• We will contact appropriate resources and fulfill your wish
